Home from Hospital Volunteer

To support vulnerable individuals leaving hospital, or those living at home with limited support, by providing short term practical assistance, typically for up to six weeks. This helps them to regain independence, maintain their wellbeing, and it reduces the risk of readmission.

When?

Flexible - there are no set hours or a minimum time commitment, however volunteer tasks must be carried out during our standard office hours (08:30 - 16:30 Monday - Thursday or 08:30 - 16:00 on Friday).

Are expenses covered?

Where a volunteer needs to use their own car, SDCVS will pay mileage at a rate of 45p per mile. Reasonable Public Transport costs can be reimbursed - to be discussed and agreed in advance. Volunteers collecting shopping for service users will be reimbursed for the cost of these items where this has been agreed in advance.

What will you be doing?

Tasks include:
- Picking up a few essential shopping items and delivering them to the service user's home.
- Collecting prescriptions or medication and delivering them to the service user's home.
- Other short-term support tasks as agreed with the Derbyshire Home from Hospital Service Co-Ordinator.

What skills are required?

It would be advantageous if you are:
- Comfortable assisting with everyday tasks.
- Consistently dependable, respecting agreed upon times for tasks to be carried out.
- Flexible in responding to different needs, environments or unexpected situations.
- Respectful of service users' privacy and handling sensitive information responsibly.
- Able to identify changes in wellbeing or environment that may require further support.

Is training provided?

Yes, full induction training is provided and all volunteers will be required to complete this before they volunteer with us. Volunteers will be offered 1-2-1 support sessions throughout the year.
